I give my permission for my child to participate in the Madison Youth Tennis Programs being held at Madison High Tennis Courts. I waive and release all rights and cleans to damage I may have against Madison Youth Tennis and the Duque Tennis Program and its agents from any injuries that my child suffered in connections with participation in this program.

I also permit my child’s image to be used in advertising purposes.
